PROGRAM
Transitional
Living Program:
Candidates are
typically one month to three years post injury. Most require, and
benefit from, traditional therapies. The goal is for these clients
to reach maximum independence in order to return to their home and
community.
Supervised
and Supported Living Programs:
Candidates are typically at least three years post injury. They
generally no longer require extensive therapies. They do require
structure, supervision of varying degrees, as well as therapeutic
recreational involvement. These individuals may also require vocational
involvement. The goal is to reach maximum independence with continued
program involvement and supervision. These individuals typically
require 24-hour supervision, decreasing, if appropriate, to fewer
hours of supervision as determined by the treatment team .
Supported
Independent Living:
Candidates are typically at least three years post injury. They
do not require supervision. They are safe in the home and community
setting. They do require occasional assistance with financing and
some home management. Group and individual counseling sessions as
well as social recreation are usually necessary. Home assistance
by the program is based upon the individual's level of independence.
Day
Programs:
Candidates are typically at least one month post injury.
They do not require a residential program, but do require a structured
half-day or full-day program to include therapies, community involvement
and interaction, therapeutic recreation and socialization, group and
individual counseling, and increased independence in home skills.
Out-Patient
Programs:
The out-patient program is open for individuals requiring traditional
individual therapies and group sessions.
